• Complete the requested information to ensure proper recognition of your gift, deposit, recording, and acknowledgment of gift per IRS rules.
• Select Options: Select type of donation (pledge or gift) and funding option.
• Gift Amount: Indicate Amount and method of payment
• Credit Card: Information when using credit card to make donation
• Matching Gift: Indicate name of Company
• Designations: Check Rutgers Club of Lehigh Valley Scholarship Fund
• Names & Address: List Donor(s) information for acknowledgement for IRS purposes
• Enclose and mail; Signed Check (if using check to make donation):
• Payee: RUTGERS UNIVERSITY FOUNDATION
• Memo: RUTGERS CLUB OF LEHIGH VALLEY SCHOLARSHIP
• Mail to:

• Be sure to include completed Pledge Form, and Matching Gift form (if appropriate).
